Documentation required from the Nurses for processing Green
Card :
IOCS will assist the Nurses at every step, namely,
Registration for NCLEX Examination, IELTS test and obtaining
VISAScreen Certification.
1. RN License from the country of origin.
2. NCLEX Certificate.
3. IELTS Score.
4. Nursing Diploma or Degree.
5. Nursing Transcript.
6. Birth certificate, Affidavit of Birth Details.
7. Passport page copies.
8. Two signed copies of DOL ETAForm9089 (provided by us).
9. Copy of Marriage Certificate (if applicable).
10. Birth Details of Spouse and / or Children (if applicable).
11. Passport details of Spouse and / or Children (if applicable).
