Up to a $5,000.00 sign on Bonus for a Physical Therapist with Home Health experience!
-The Physical Therapist will provide physical therapy services in client's homes.
-Performs comprehensive and timely evaluations of homebound individuals.
-Assist the physician in evaluating the client's level of function, helping to develop and revise the plan of treatment.
-Demonstrates appropriate treatment planning based on home health standards of care for physical therapy services.
-Adheres to hospital/ departmental safety /environment of care education requirements.
-Demonstrates knowledge and skills needed to provide appropriate levels of service to customers in specific age groups.
-Supports and complies with hospital's mission, vision, and values.
Detailed Description
The physical therapist will provide physical therapy services in client's homes, in accordance with applicable laws, rules, agency policies and the client's plan of treatment. In addition, the physical therapist will perform comprehensive and timely evaluations of homebound individuals referred to the Home Health Agency as well as assist the physician in evaluating the client's level of function and help to develop and revise the plan of treatment. The successful candidate will demonstrate appropriate treatment planning based on home health standards of care for physical therapy services and adhere to hospital/ departmental safety /environment of care education requirements. In addition, the physical therapist will demonstrate knowledge and skills needed to provide appropriate levels of service to customers in specific age groups and will support and comply with our client's mission, vision, and values.

| Skills Required: |
-Graduate from an accredited school of Physical Therapy.
-Currently licensed to practice as a Physical Therapist in the State of Oregon.
-Current Oregon drivers License, (dependable car).
-American Heart Association Basic Life Support (BSL), Health Care Provider card.
-Physical Therapist with 3-5 years recent work experience.
-One year of Home Health experience preferred.
-Demonstrate ability to work in client's homes of various socioeconomic levels with possible exposure to pet, smoke, etc.
-Demonstrable priority setting ability and organizational skills.
-Effective written and oral communication skills.
-Proactively adapts to change.
-Demonstrate successful client-centered outcomes while participating as a member on an interdisciplinary team.
