| Job Description: |
The Physical Therapist is a qualified health care professional who directs, supervises, evaluates, and provides physical therapy services to clients in the home as prescribed by the attending physician. He/she is responsible for: (1) providing skilled physical therapy care to patients with impairments, functional limitations, disabilities, or changes in physical function resulting from injury, disease, or other causes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care.

| Skills Required: |
Qualifications:
* Graduation from a physical therapy school accredited by the Commission on Accreditation in Physical Therapy Education.
* Licensed or registered as a Therapist in the state(s) in which he/she is required to practice.
* Experience A PLUS but New grads welcome!
