A rapidly growing home health agency is seeking a Registered Nurse to visit clients’ homes providing direct assessments for Medicaid Waiver & Personal Care Programs. Registered Nurse (RN) develop and manage an initial Plan of Care (POC), and help individuals and groups take steps to improve or maintain health. You will be responsible for initial evaluation of a clients needs, hospital follow-up visits & 6 month assessments. Paid training is available. Position available for full or part time and you have the freedom to schedule your own appointment windows or our friendly office staff can schedule for you.
