| Job Description: |
The Recreation Therapist shall be responsible for the provision and quality of recreational services provided to residents of the facility.
Work Experience: Entry level. Experience preferred in recreation related area.
Education: BA in Therapeutic Recreation or related field.
A. Assessment
1. Interviews resident and/or significant other.
2. Completes appropriate assessment per documentation policy and procedure, and records this in the medical record.
3. Consults with the interdisciplinary team to obtain complete/share information.
B. Clinical Responsibilities
1. Establishes and implements individualized care plan and treatments for each resident.
2. Documents plan and ongoing progress notes in the medical record.
3. Documents MDS/RAI accurately, completely, and timely.
4. Attends interdisciplinary care conferences as an active participant.
5. Transports residents to Therapeutic Recreation programming as necessary.
C. Professional Responsibilities
1. Participates in Performance Improvement projects as assigned.
2. Attends in-services, educational seminars, and workshops as appropriate.
3. Maintains adequate level of department supplies.
4. Supervises volunteers as determined by Director.
5. Drives facility van as needed for out trips.
