| Description: |
An Intake Coordinator is needed to assist clients with completing applications for program enrollment, perform biopsychosocial assessments, complete case notes and referrals, and do required follow-up. The Intake Coordinator will also perform data entry and track client information and enrollment data for funder mandated reports.
Prior experience in case management is an asset. Excellent communication skills are required. Must be able to work independently, have strong attention to detail, and proficient writing skills. The ability to work one-on-one with clients performing interviews and assisting them in completing the application process is a must. A familiarity with human services assessment tools is preferred.

| Capacités Requises: |
A bachelor’s degree or higher in the human services field is required. Must be computer literate in MS Word and Excel (PowerPoint and Access helpful, but not required). Prior experience using URS or AIRS is desirable. A working knowledge of HIV infection, mental health, and substance use issues is preferred. Applicants with a valid NYS Driver’s License are preferred. Bilingual English/ Spanish is helpful, but not required.
