This leading Organisation is seeking an experienced Clinical Pharmacist for a permanent role based in Qatar.
In this role you will be responsible for designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes. Ensures implementation of the pharmaceutical care plan by assuring the timely distribution of medications to patients. Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.
Your main duties will include:
Assume responsibility for patient-specific pharmacotherapy plan to ensure optimal outcomes.
Actively participates in patient care rounds with other members of the healthcare team.
Assists medical staff with the selection of drug therapy and patient monitoring. Reviews and interprets medication orders for clarity, accuracy, rationality, compatibility, feasibility, and suitability for the patients specific needs.
Solicits opinions and recommendation from staff pharmacists and healthcare professionals to improve service.
Liaises with other healthcare professionals to coordinate pharmacy activities and to assists in the resolution of interdepartmental problems. Ensures that medical and nursing staff are aware of assistance which can be given by the Pharmacy.
Acts as a drug information and education resource for patients, colleagues, medical and nursing staff / other healthcare professionals.
Prepare to perform on-call duties.
Assures adherence to the departmental policies and procedures and provides monthly reports ton clinical activities and other relevant workload statistics.
Participates in the departmental performance improvement, and educational activities.
Follows all hospital related policies and procedures.
Review Pharmacy Policy and Procedures and makes revisions as required.
Familiar with Jcaho / Jcia Standards for quality improvement and prepare for accreditation.
To be considered you will be degree qualified with four years of hospital pharmacy experience including two years in clinical capacity.
Other requirements include good aseptic technique in preparing IV admixture and experience in preparation of JCIA materials, Pharmacy Quality Improvement program.
