Hospice Nurse at Fraser Health

We are seeking a dedicated Registered Nurse or Registered Psychiatric Nurse with at least two years of medical nursing experience. In this role, you will assess and implement individualized care plans for hospice patients, ensuring their physical, emotional, and psychosocial needs are met. Collaborate with a multidisciplinary team that values diversity and mutual respect.

Key Responsibilities:

• Evaluate nursing care through assessments and interventions

• Collect data on patient conditions to inform care

• Collaborate with the health care team on care plans

• Document assessments and nursing actions timely

• Participate in educational activities as a preceptor
Requirements:

• Approved School of Nursing graduation

• Two years of related nursing experience

• One year in hospice care preferred

• Current BCCNM registration

• Ability to work both independently and in a team
