Pharmacist (Primary Care) - Amman Tawe Partnership (Gwaun Cae Gurwen)
To provide high-quality, patient-centred clinical pharmacy services across Amman Tawe Partnership, supporting safe, effective and optimised prescribing. The post holder will work as part of a friendly and supportive multidisciplinary team, contributing to excellent patient care, medicines optimisation, workforce education, and quality improvement across primary care.
Main duties of the jobProvide expert clinical pharmacy to improve medicines optimisation, patient safety and prescribing quality. Undertake medicines reconciliation following hospital discharge/outpatient correspondence and manage repeat prescribing systems. Respond to medication queries from patients, clinicians, community pharmacies and secondary care.
Complete structured annual medication reviews, prioritising patients with polypharmacy, frailty, long-term conditions and high-risk medicines. Support shared decision-making, adherence and deprescribing where appropriate.
Provide anticoagulation support including DOAC/NOAC initiation/monitoring/annual reviews, and INR dosing/warfarin management in line with local protocols. Support safe use and monitoring of high-risk medicines.
Manage shared care agreements, ensuring monitoring and interface communication with secondary care. Deliver medicines optimisation for care home and complex frailty patients, reducing inappropriate polypharmacy and medicines-related harm in collaboration with the MDT.
Lead and contribute to QI and Prescribing Management Scheme (PMS) projects, audits and service evaluations, using prescribing data to improve outcomes.
Support the practice as a training environment by contributing to education and supervision of GP trainees, medical students and PA students.
Maintain accurate clinical records, comply with governance requirements, participate in audit/service development, and maintain GPhC registration and CPD.

Job responsibilitiesProvide expert medicines management support within GP practices
Undertake medication reconciliation following hospital discharge and outpatient correspondence
Support safe, effective and cost-efficient prescribing in line with NICE, AWMSG and Health Board guidance
Manage and support repeat prescribing systems
Respond to prescribing and medication queries from patients, clinicians, community pharmacies and secondary care
Clinical Medication Reviews
Undertake structured medication reviews (SMRs) and annual medication reviews
Focus on patients with:
Polypharmacy
Frailty
Long-term conditions
Support shared decision-making, deprescribing where appropriate, and improved adherence
Anticoagulation & High-Risk Medicines
Manage patients prescribed DOACs / NOACs, including:
Monitoring
Annual reviews
Provide INR dosing and warfarin management in line with local protocols
Support safe use of high-risk medicines (e.g. opioids, lithium, methotrexate)
Shared Care & Interface Working
Manage and support shared care agreements
Review and action medication‑related hospital correspondence
Act as an interface between primary care, secondary care and community pharmacy
Care Home, Frailty & Complex Care
Support medicines optimisation for care home residents
Undertake frailty‑focused medication reviews
Reduce inappropriate polypharmacy and medicines‑related harm
Work closely with GPs, care home staff and MDT colleagues
Quality Improvement & Prescribing Projects
Contribute to and lead Quality Improvement (QI) projects
