The starting salary ranges from $45,000 to $50,000 annually, based on skill and experience level.
ESSENTIAL JOB FUNCTIONS- Works with the full family in a collaborative and supportive manner to help, motivate, and inspire individual and family goal setting while allowing the family to drive the work.
- Has a deep respect for a family’s expertise, which is fundamental to establishing trust.
- Helps the family identify strengths and then work with them to utilize and build those strengths.
- Uses trauma‑informed practices.
- Provides services according to values and principles for family‑centered coaching.
- Completes intakes and monitors family goals and activities.
- Completes senior assessments and monitors goals, activities, and referrals.
- Provides follow‑up to ensure family goals are progressing or achieved.
- Focuses services on fostering economic independence, increasing earning power, supporting academic achievement, and enhancing stability and quality of life.
- Coordinates events and activities within the region to meet client needs and engage individuals, seniors, households in services.
- Addresses housing‑related issues including lease compliance and referrals from Property Management.
- Assists families with meeting site‑based requirements and CHA work requirements.
- Provides support services including transportation, childcare assistance, clothing and uniform assistance.
- Records family progress in the HMIS and case files.
- Handles emergency and crisis situations with residents.
- Works within contract, grant, and program requirements, meeting timelines and reporting obligations.
- Utilizes a team approach and collaborates with other program staff in planning and implementing services.
- Grows in own professional development through supervision and opportunities within and outside the agency.
- Works effectively with diverse staff and service populations, providing exceptional customer service.
- Other duties as assigned.
SKILLS AND ABILITIES
- Demonstrated ability to render adequate service to clients and to organize and manage jobs efficiently.
- Mobility to work with a community‑based caseload.
- Ability to provide assigned community and home‑based services.
- Ability to work effectively with diverse individuals and groups.
- Must be highly organized and proficient with Excel for data tracking.
- High School Diploma or equivalent (required).
- At least 2 years’ experience in case management, including coordination of services and housing needs (required).
- Bachelor’s degree in social work or a related field from an accredited institution (preferred).
- Successfully pass all relevant screenings and criminal background checks.
- Driving for work required with a personally owned vehicle; travel between sites required.
- Must sit and stand for extended periods, talk, use hands and fingers, occasionally stand, bend, stretch, stoop, kneel, and crouch; requires visual acuity for data preparation and computer use.
- Medical, dental, and vision coverage.
- Contributions of $2,000 to individual and $4,000 to family Health Savings Accounts, pro‑rated to benefits start date.
- Teladoc services for general medicine and behavioral health.
- Ascensus 401(k) plan with a 4% employer match, 100% vested on day one of eligibility.
