A community outreach company is seeking a FT/PT Paraprofessional due to growth and expansion. This is a non-exempt salary position. The position requires providing services such as case management, outreach interventions, direct treatment and rehabilitative and supportive services in a community setting or center to clients in their natural environment. All applicants are required to work as a member of a clinical team to assess client needs and wants, as well as development of individual service plans. Candidates will work closely with hospitals, jails and shelters, as well as meeting with the clients in the community in accordance with established policies, procedures and clinical protocols to provide supportive counseling skills. Timely development and implementation of the client’s service plan is imperative. Coordinate with all parties involved in the client’s treatment according to policy and procedure via face-to-face, telephone or written communication. Exceptional interpersonal communication skills and excellent writing skills are mandatory. Candidate will be responsible for attending team meetings and meeting deadlines on a daily basis. Candidate must insure that the client is connected to community services and light transportation is required.
- Bachelors/Masters Degree in Social Science, Psychology, Sociology, Counseling (helping field)
- Ability to maintain good client relationships
- Excellent written and verbal communication skills
- Ability to stay organized, prioritize and meet deadlines
- Ability to work as a member of a team and individually
- Valid Driver’s License and reliable transportation are a MUST

We utilize the "Wrap-around Philosophy" which consists of identifying the community services and support that the client may need and providing them with those services for as long as may need them. The Wraparound process is designed to adhere to the needs of the client who is at risk of repeated hospitalizations. Treatment and services are driven by the Individualized Service Plan developed with the client, team members and other professional services.
