Job Summary

The Advanced Practice Provider (NP or PA) functions as a care provider within the Oncology practice. This requires collaboration with the interdisciplinary team to provide assessment and therapeutic management of patient/family needs. Care is provided to hematology oncology patients in ambulatory and hospital settings. The APP will work alongside the physicians and the clinical team within the multidisciplinary oncology service line, designed to meet the needs of cancer populations within the continuum of care;

cancer screening, diagnosis, treatment, survivorship, etc. The major responsibilities include expanded patient assessment, triage, diagnosis, coordination and documentation of care.

Essential Functions
  • Performs comprehensive initial and episodic health histories and physical exams; in order to identify existing or potential health problems
  • Initiates and implements an individualized nursing and medical plan of care in collaboration with physicians and members of interdisciplinary team
  • Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician
  • Provides follow-up care including acute and routine examinations
  • Assesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physician
  • Monitors and adjusts patients medication according to protocol or medical plan of care including adequate pain control
  • Implements nutritional therapies, as appropriate
  • Triages patient problem calls in office, which requires medical attention
  • Orders appropriate laboratory and diagnostic procedures
  • Recognizes normal and abnormal laboratory findings and the pathology involved
  • Documents information in patients charts following documentation standards
  • Provides health education to the patient and family (e,g, management of side effects, anticipatory teaching)
  • Manages medical emergencies in office, as indicated, and according to standing orders
  • Provides monitoring and continuity of care between health care settings
  • Provides patient and family counseling and reassurance in regard to psychological and or psychosocial issues
  • Initiates proper referrals to private or community agencies
  • Acts as a resource and teacher for other members of the health care team
  • Shares knowledge and expertise with staff
  • Collaborates with Surgeons, radiation oncology, inpatient nursing and medical staff as a partner to coordinate continuity of care
  • Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicated
  • Supports the research/clinical trials mission by collaborating with the research team
  • Identifies and screens patients for appropriate eligibility for protocol study participation and aids in the accumulation and recording of date regarding patients on protocols
  • Reviews and audits charts routinely
  • Performs other related duties as assigned
Job Requirements
  • Registered Professional Nurse Or Physician Assistant with current NJ state licensure,
  • NP/PA Certification/License
  • CDS and DEA required
  • BLS certification required
  • ACLS certification preferred
  • Oncology experience highly preferred
  • Oncology nursing certification (OCN, AOCNP) preferred,
Hours
  • Monday Friday 8A 5P; support both inpatient and outpatient patient care needs
  • Some call coverage may be required
