Leaving Care Personal Advisor Department:
Children’s Services Job Purpose
To provide consistent, practical, and emotional support to care‑experienced young people aged 16–25 as they transition from care into independent adulthood. The role focuses on building trusting relationships, promoting independence, and helping young people achieve positive outcomes in housing, education, employment, health, and wellbeing.
Key Responsibilities- Act as the key worker for care leavers, supporting them from age 16 up to 25
- Develop, review, and implement Pathway Plans in partnership with young people and professionals
- Support young people to develop independent living skills, including budgeting and managing accommodation
- Work with housing providers to secure and sustain suitable accommodation
- Support access to education, training, employment, and health services
- Advocate for young people and ensure their voice is central to planning and decision‑making
- Work collaboratively with social workers, housing, education, and external agencies
- Maintain accurate case records and complete administrative and safeguarding documentation
- Participate in service development, training, and multi‑agency working
- Promote equality, safeguarding, and the Council’s corporate parenting responsibilities
- Experience working with vulnerable young people (ideally 2+ years)
- Strong understanding of Leaving Care, safeguarding, and child protection legislation
- Knowledge of trauma, adverse childhood experiences, and their impact on development
- Ability to build trusting relationships and manage a caseload effectively
- Strong communication, organisation, and assessment skills
- Experience working in partnership with multi‑agency teams
- Ability to work independently, flexibly, and in challenging situations
- NVQ Level 4 in Childcare/Social Care or equivalent, or a relevant degree
- Full UK driving licence and access to a vehicle
- Willingness to work flexibly, including outside normal hours when required
- Enhanced DBS clearance
