We are growing our Family Practices and seeking experienced APP's.
Advanced Practice Provider will see patients in a primary care clinic setting. Candidates must be licensed to see patients of ALL ages and be comfortable with Sick, Return, Annual Wellness/Physical, and Emergent Follow Up type visits. Clinic Schedule is open Days, Monday - Friday. 1 weekend morning a month is required. Specific schedule would be negotiated upon hire.
Responsibilities- Advance Practice Professional will provide health care and specialized health services to patients and families enrolled in Primary Care Medicine.
- The APP will coordinate care of this patient population in collaboration with all members of the health care team.
- This position will be 100% outpatient.
- APP will provide outpatient medical care by taking patient history, performing physical examinations, orders, and interpreting tests, making diagnoses, and instituting appropriate treatment programs.
- Independently evaluates patients in the clinic.
- Ordering/interpreting testing and lab results, modifying plan of care to address results.
- Provide clinical expertise and direction to support patient medication management.
- Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians.
- Streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external team members and other ambulatory care providers.
- Perform other related duties as required to assure patient safety and satisfaction.
- Participate in the development and presentation of educational experiences for patients, families, and health care professionals.
Provides inpatient and/or outpatient medical care. Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs. Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently. Specific procedures are individually credentialed as indicated by training and experience.
Education and Experience- Minimum of 1-2 years’ experience as an APP.
- Bachelor’s Degree or Completion of an AMA-approved Physician Assistant program.
- Michigan state licensure and continued certification by the National Commission on Certification of Physician Assistants (NCCPA).
- Current ACLS or BLS as required.
- Current ATLS/PALS provider status as required.
- Requires master’s degree in nursing; licensure as a Certified Family Nurse Practitioner; licensure as a Registered Nurse and certification in one of the following: ANA certification exam in Family specialty field;
National Certification Corporation (NCC); and/or certification from Michigan State Board of Nursing as a Nurse Practitioner (NP) - OR - requires a bachelor's degree, completion of an AMA-approved Physician Assistant program, Licensure as a Physician Assistant.
