Position: Oncology Nurse Navigator Breast Oncology 40 Hours

Position Overview

The Oncology Nurse Navigator (ONN) uses oncology‑specific clinical knowledge to offer individualized guidance to patients, families, and caregivers, fostering independence during a complex cancer journey. The ONN assesses patient needs, provides education, and facilitates timely quality health and psychosocial care throughout all phases of the cancer continuum.

Primary Duties and Responsibilities
  • Participate in day‑to‑day operations including patient education and call triage.
  • Participate in program evaluation and development of strategic goals aligned with DFCI mission.
  • Participate in QA/QI projects for the program.
  • Support research activities of the program.
  • Demonstrate professional integrity, honesty, and accountability.
  • Deliver expert clinical care through evidence‑based practice partnerships.
  • Assume nursing care responsibilities for a defined group: assessment, education, coordination, and documentation.
  • Facilitate individualized care considering functional status, culture, health literacy, psychosocial, reproductive/fertility, and spiritual needs.
  • Support smooth transition of patients from active treatment to survivorship, chronic cancer management, or end‑of‑life care.
  • Assist patients with treatment goals, advanced directives, palliative and end‑of‑life concerns.
  • Act as liaison between patients, families, caregivers, and providers to optimize outcomes.
  • Advocate for patient‑centered care and shared decision making.
  • Provide psychosocial support and facilitate appropriate referrals.
  • Contribute to ONN program development, implementation, and evaluation.
  • Track and monitor metrics and outcomes, collaborating with administration.
  • Serve as primary nursing contact for care delivered within physician practice settings.
  • Engage in collaborative decision making using data expertise.
  • Manage patient care across the continuum, including new patient calls and appropriate referrals.
  • Work collaboratively as an effective member of health care team.
  • Monitor environment of care for patient safety and regulatory compliance.
  • Maintain evidence‑based clinical practice aligned with nursing policies and standards.
  • Provide care based on age‑specific needs.
  • Adhere to the Code of Ethics for Nurses and accepted standards of professional practice.
  • Foster an environment sensitive to diverse populations’ needs.
Professional Development
  • Identify areas for professional growth.
  • Formulate professional goals, objectives, and methods to achieve them.
  • Serve as representative on Nursing and Institute committees.
  • Support research activities within Nursing and Patient Care Services.
  • Meet all annual evaluation requirements and competencies.
  • Collaborate in preparing professional reports, articles, and presentations.
  • Pursue active membership in local and national professional organizations.
  • Commit to continuous learning and improvement.

    Minimum Job Qualifications
    • Education:

      Associate Degree, Nursing. Baccalaureate degree strongly preferred;
      Associate must complete BSN within 4 years.
    • Experience:

      1 year of recent RN experience; ambulatory and/or oncology experience preferred.
    License/Certification/Registration Required
    • Licensed RN in Massachusetts or New Hampshire.
    • AHA BLS required; ACLS accepted.
    • Professional nursing certification preferred (OCN, BMTCN, CPHON, AOCNP, CRNI, RN‑BC).

    Yes. Requires understanding of special needs and behaviors across age groups (young adults to geriatrics).
