Prof RN - Case Manager, Per Diem
Job Profile SummaryThis role provides professional and nonprofessional nursing care services in accordance with physician orders. It includes assessing, planning, implementing, and evaluating the care needs of a designated group of patients or providing nursing care and identified clinical support tasks within the scope of practice to achieve the patient’s plan of care as directed. The role requires a nursing license and may direct other lower‑level professionals or manage processes and programs.
Most of the time is spent overseeing the design, implementation, and delivery of processes, programs, and policies using specialized knowledge typically acquired through advanced education. The role is a senior‑level individual contributor responsible for managing projects/processes, working independently with limited supervision, coaching and reviewing the work of lower‑level professionals, and resolving difficult and complex problems.
The professional registered nurse possesses and applies the skills and knowledge necessary for the delivery and management of patient care from admission through discharge. The nurse is knowledgeable of current trends in healthcare, including advances in technology, pharmacology, treatment modalities, quality, and patient safety. The nurse serves as a patient and family advocate while facilitating a cooperative and collaborative environment among all health care providers.
The nurse is responsible for planning, delivering, and managing patient/family‑centered care using the nursing process and adhering to standards of practice, research, evidence‑based practice, and best practices.
- Registered Nurse (RN) license or license‑eligible
- Basic Life Support (BLS) certification
- Three (3) years of Case Management experience
- Bachelor of Science in Nursing (BSN)
- Specialty nursing certifications
$43.01 - $70.95 per hour
