Zintri in Breda is op zoek naar een ambulant begeleider die ondersteuning biedt aan cliënten met complexe psychische problematiek in hun thuissituatie. Je stelt zorgplannen op en evalueert deze, terwijl je rapportages schrijft en samenwerkt met cliënten en hun netwerk.
Je bent minimaal Mbo-4 opgeleid, bij voorkeur Hbo, en hebt kennis van de psychiatrie. Wij bieden een parttime of fulltime dienstverband, afhankelijk van je voorkeur, met een salaris tussen de €2804 en €3803 per maand op basis van een fulltime dienstverband.
