Location: Brooksville, Florida

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $53 per hour

• Coordinate seamless care transitions for patients across the continuum
• Collaborate with physicians, nurses, and social workers to create individualized discharge plans
• Advocate for patients and families to ensure they have the resources they need
• Utilize Meditech and Midas systems to track and document case management activities
• Help patients navigate complex healthcare systems with compassion and expertise

• Active RN license in Florida or compact license
• Minimum 1 year of acute care case management experience
• Strong organizational and communication skills
• Experience with Meditech and Midas preferred
• HCA experience is a plus

• Acute care case management expertise
• Patient advocacy and care coordination
• Discharge planning and resource navigation
• Electronic medical record proficiency
• Collaborative teamwork across disciplines
