Licensed Practical Nurse (LPN)

A Licensed Practical Nurse provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.

• Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient.
• Performs patient care considering needs specific to the standard of care for patient age; includes geriatric and general inpatient rehabilitation patients.
• Under the supervision of the RN, assists with patient assessments and reassessments as per policy, identifies and initiates appropriate rehabilitation nursing interventions, and performs timely and accurate QI assessments.
• Carries out the plan of care as indicated by patient needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient condition and individual needs.
• Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
• Demonstrates ability to assess and reassess pain, utilizes appropriate pain management techniques, and educates the patient and family regarding pain management.
• Other duties as assigned.

Strong interpersonal and documentation skills required. Familiarity with rehab documentation systems is preferred.

• Licensed Practical Nurse license in the state of Indiana.
• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification; ACLS preferred.
• Excellent oral and written communication and interpersonal skills.
