Position: Hospice Registered Nurse - RN | Horizon Home Health & Hospice |Caldwell, ID

Qualifications

  • Registered Nurse (RN) licensed in Idaho.
  • Valid driver's license and auto insurance.
  • Home Health experience preferred.
Key Responsibilities
  • Complete initial assessment of patient and family to determine home care needs.
  • Provide a comprehensive assessment and history of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs.
  • Initiate and revise the care plan as patient status changes.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a care plan that establishes goals based on the nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative actions.
  • Include the patient and family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician.
  • Provide counseling to patient and family to meet nursing and related needs.
  • Provide health care instructions per assessment and plan of care.
  • Identify discharge planning needs and implement prior to patient discharge.
  • Act as case manager when assigned and coordinate care for the caseload.
  • Adhere to organization’s documentation, care procedures, and professional conduct standards.
  • Prepare clinical notes and update the physician at least every 60 days.
  • Communicate with the physician regarding patient needs and condition changes; obtain and receive orders as required.
  • Coordinate with community health professionals to align the care plan.
  • Participate in on-call duties as per policy.
  • Ensure arrangements for equipment and other necessary items and services.
