Position: Easter Seals Alberta Social Worker Role

Reporting to the Program and Resource Manager, you'll help individuals and families navigate disability services like PDD and FSCD. Your expertise in needs assessments will be invaluable in creating personalized plans that promote inclusion and independence.

Key Responsibilities:

• Connect clients to community resources and supports

• Co-create individualized service plans for clients

• Implement key planning tools like Transition to Adulthood Plans

• Monitor and enhance service delivery processes

• Engage with community partners to improve service coordination

Requirements:

• Degree in Social Work or a related field

• Minimum 2–3 years of relevant experience

• Solid understanding of disability support systems

• Familiarity with training and facilitating educational sessions

• Commitment to diversity and inclusion strategies
