Chronic Disease Management Nurse

Location: Western Saskatchewan (Kindersley)

Excel as a Chronic Disease Management Nurse at Eston Health Centre, providing patient-centered care in a part-time role with flexible shifting. This position focuses on collaborative care.

As part of the Chronic Resident Unit team, your main focus will be on coordinating and delivering quality nursing services to patients facing chronic health conditions. A minimum of one year’s nursing experience and a Bachelor’s degree are essential for this role. You'll work independently and with a team to promote health and evaluate ongoing initiatives.

Key Responsibilities:

• Administer ongoing nursing care for chronic patients

• Collaborate with healthcare professionals for patient care

• Initiate community support programs related to health

• Conduct patient assessments using critical thinking

• Lead quality improvement projects within the clinic

Requirements:

• Bachelor of Science in Nursing (BScN)

• Must register/eligible with CRNS

• One year of experience in a clinic environment

• Basic Life Support Certificate (BLS)

• Experience in community health initiatives is an asset
