Southeast Advocates is seeking a Transition Consultant to support youth with disabilities transitioning from high school to post‑secondary education, employment, or other community pathways. The position works directly with students within school settings and in the community. Regular travel to schools in Weyburn, Estevan and surrounding areas is required. Approved travel expenses will be reimbursed.

Duties

Provide one‑on‑one and group support to youth with disabilities.

Develop and implement individualized transition plans in collaboration with students, families, school staff and community partners.

Support career exploration, employment readiness, life skills and goal setting.

Liaise with employers, post‑secondary institutions and service providers.

Maintain accurate documentation and case notes.

Travel regularly to assigned schools and community locations.

Qualifications

Post‑secondary education in Human Services, Education, Social Work, Disability Studies or related field, or equivalent experience.

Experience working with youth and/or individuals with disabilities.

Knowledge of transition planning and employment supports is an asset.

Strong communication, organization and relationship‑building skills.

Ability to work independently and as part of a team.

Additional Requirements

Valid drivers licence and access to a reliable vehicle.

Ability to travel within the Southeast region (mileage reimbursed).
