The RN Case Manager is responsible for advocating for patients, coordinating high‑quality care transitions across acute, post‑acute and home settings, and collaborating with a multidisciplinary care team to achieve positive patient outcomes.
Duties & Responsibilities
- Manage and plan transitions of care, discharge, and post‑discharge follow‑up for patients admitted to high‑volume/high‑priority hospitals.
- Establish trusting relationships with patients, families, and caregivers and serve as their primary advocate.
- Collaborate with clinical staff to develop, execute, and monitor individualized care plans and goals.
- Coordinate referrals to preferred acute care providers and facilitate communication with PCPs and transitional care physicians.
- Integrate social services and case management functions across pre‑acute, ER, acute, and post‑acute settings.
- Report and respond to variations in patient condition via email, dashboard, or other approved channels.
- Provide high‑intensity engagement, education, and family conferences to optimize resource utilization.
- Address advanced care planning, advance directives, and complex psychosocial needs through referrals to social workers.
- Maintain accurate clinical and progress notes, submit required documentation timely, and participate in projects and surveys.
- Conduct medical record reviews, investigate adverse occurrences, and educate staff on resource utilization.
- Registered Nurse (RN) with valid license in Illinois.
- Experience in case management, acute or community nursing preferred.
- Strong interpersonal and communication skills with ability to work autonomously.
- Critical thinking and problem‑solving abilities.
- Proficiency with Microsoft Office Suite (Excel, Word, PowerPoint, Outlook).
- Fluency in English; bilingual preferred.
- Willingness to travel locally, regionally, and nationwide up to 10% of the time.
- Knowledge of nursing and case management theory and practice.
- Knowledge of patient care charts, histories, and clinical documentation procedures.
- Knowledge of community health services, social services support agencies, and networks.
- Organizing, coordinating, and scheduling skills.
- Ability to communicate technical information to non‑technical personnel.
- Strong decision‑making and resource‑utilization skills.
- Legal and regulatory compliance understanding.
Hourly rate: $36.90 – $52.70 (base). Final compensation may include bonuses or commissions based on experience, education, geography, and other factors.
