Nurse Practitioner; Weekend​/Part Time

Provider Partners is seeking a flexible and dedicated Weekend Nurse Practitioner. The Weekend Nurse Practitioner is responsible for coordinated and comprehensive care of its Provider Partner Health Plan (PPHP) members at designated skilled nursing facilities (located in Oak Lawn and Palos Heights) on Saturdays and Sundays for 8 hours a day (the 8-hour shift can be flexible but must cover part of day and evening shift;

ex 8a-4p, 9a-5p, 10a-6p). The Weekend NP must be present in facility for the entire 8-hour shift.

Under the general direction of the Clinical Operations Manager/Director, The Weekend Nurse Practitioner works in collaboration with the member’s primary care physician and PPHP Primary NP to deliver exception health care. The Weekend Nurse Practitioner is the medical liaison to the interdisciplinary staff and allied health providers and communicates effectively to assure seamless delivery of services.

As a key member of the clinical team in the skilled nursing facility, the Weekend Nurse Practitioner provides preventive care, diagnoses both acute and chronic illnesses, orders and interprets diagnostic tests, prescribes medications, refers to specialists as appropriate, establishes a plan of care with short-term and long-term goals, and is a resource to members, families, and staff.

• Demonstrates both autonomy and proficiency in performance of comprehensive health assessments and management of the chronically ill.
• Collaborates with member’s primary care provider/interdisciplinary care team to create an individualized care plan focused on primary care, prevention, early detection and timely intervention.
• Adheres to standards of documentation and collaboration as directed by the PPCM ISNP Model of Care.
• Follows practice guidelines according to state-specific nursing scope of practice
• Effectively communicates with all members of the interdisciplinary care team and facilitates an effective, medically-appropriate plan of care to avoid unnecessary hospitalizations, duplication of testing and fragmentation of care.
• Participates in clinical rounds to review members who are skilled, hospitalized, and with acute change in conditions.
• Participates in quality initiatives related to ISNP &/or PPCM requirements.

• Minimum of 2 years clinical care experience in geriatrics, long-term care, or internal medicine.
• Familiarity with ISNP models of care and value-based care principles is preferred.
• Proficiency in basic computer skills, virtual meeting platforms, Microsoft and LTC clinical EMRs.
• Excellent communication and documentation skills.

• Current certification as an Adult, Geriatric or Family Nurse Practitioner in the state of practice.
• Current unrestricted RN license for the state in practice.
• Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
• Meets PPCM credentialing requirements.
• Holds current Basic Life Support certification.
• Holds valid driver’s license and car insurance and has reliable transportation
