* Must pass a pre-employment drug screen and subject to random drug screens

Program Specific Job Description:

1) Direct Clinical Care & Case Management

Provides professional therapeutic services to persons with mental illness and/or addictive disease who have Medicare coverage or whose disability is intractable or difficult to manage. Arranges appointment schedule at optimal intervals to maximize use of work time. Conducts face-to-face comprehensive assessment to identify problems and service needs. Provides individual, family or group therapeutic counseling and/or educational and skill development sessions as indicated in the service plan. Monitors client progress and compliance with the service plan.

2) Crisis Management

Provides crisis intervention services to existing and walk-in clients as needed. May provide on-call, after-hours services on a rotational schedule. Assesses consumer's need for emergency admission certification. Executes the legal forms required to refer the client to an emergency receiving facility for evaluation of the need for involuntary commitment to a hospital or crisis intervention facility. Arranges transfer of the client to the receiving facility.

3) Clinical Documentation

Writes individual service plans that meet Medicaid and other required standards for diagnoses, content, justifications for treatment, evidence of client input, and treatment time frames. Writes service plans that are clear, concise, complete, grammatically correct, timely, and properly coded. Secures or provides necessary signatures required to secure authorization for services. Follows all Medicaid, state, insurer, and other regulations related to the credentials required to provide authorized services. Writes timely progress notes that relate directly to the treatment outcomes described in the service plan. Assembles, analyzes, and interprets new/updated information about the client in order to reassess treatment needs. Identifies unmet needs, reviews available resources, and modifies the treatment plan as indicated. Requests treatment authorizations and re-authorizations within required time frames. Provides and bills only those client services that are authorized in the service plan.

4) Case Management, Referrals, Collateral & Treatment Team Contacts

May serve as case manager to facilitate and ensure communication, linkage, and continuity of care between clients, physicians, other organizational components, community resources and service providers. Establishes linkages and coordination of services with community agencies. Develops and maintains cooperative working relationships with community service providers. Informs clients and families of the availability of community advocacy and support groups.

Individual Job Description:

* Flexible 8 hour shifts available between 8am - 7pm Monday through Friday
* On-call (after hours) rotation
* Working with SA & MH Populations

Qualifications:

* Minimum Training & Experience:
* Master's degree from an accredited program
* Georgia professional license as either LAPC, LMSW or LPC, LCSW
* ONE year of supervised experience in mental health and addictive diseases
* Must pass criminal records fingerprint check.

Master's degree in a related area from an accredited college or university AND eligibility to be licensed AND Two years of supervised experience in the area of assignment.
