Position: Hospice Registered Nurse (RN) Case Manager

Looking for purpose and balance? Join Journey Care Hospice as an RN with pay up to $97K, strong benefits starting 1st of the month after hire, and a consistent Monday–Friday schedule—with minimal weekends.

Hospice Registered Nurse (RN)

Journey Care Hospice

Schedule: Monday–Friday, 8:30 AM–5:00 PM
Every 12th weekend (Saturday and Sunday)

Territory: Crystal Lake, IL

Compensation: Up to $97,000 annually
Final offer based on years of experience

What You’ll Do
  • Serve as the primary nurse for assigned patients and families, including assessment, eligibility determination, admissions, care planning, implementation, and evaluation
  • Assess physical, psychological, social, and spiritual needs to ensure holistic, patient-centered care
  • Collaborate with physicians, interdisciplinary care team members, and community partners to coordinate optimal care
  • Conduct regular patient visits to reassess needs and update care plans
  • Perform clinical pronouncement of death and provide compassionate support to families
  • Maintain timely, accurate patient documentation including charts, progress notes, and care plans
  • Supervise and document care provided by Home Health Aides in compliance with federal, state, and company guidelines
Qualifications
  • Graduate of an accredited nursing program
  • Current Registered Nurse (RN) license in the state of practice
  • Current CPR and First Aid certification
  • Knowledge of quality assessment and assurance procedures
  • Valid driver’s license and proof of insurance
