Position: Nurse Practitioner - Family Practice - $105K-150K per year

Job Summary

Chen Med is seeking a Nurse Practitioner in Family Practice for a position in Decatur, Georgia. The NP will provide direct patient care in medical centers, nursing homes, skilled nursing facilities, and home settings, performing assessments, developing care plans, and documenting all interventions as per Chen Med standards.

Responsibilities
  • Independently assess acute and non‑acute clinical problems, perform physical assessments and patient histories, and develop patient management plans.
  • Develop nursing plans of care, provide counseling and education to patients and families, and serve as patient advocate.
  • Write admissions, transfers, and discharge orders; order labs, diagnostics, medications, and treatments.
  • Refer patients for specialty consultations as indicated (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary).
  • Document progress notes and summaries in depth.
  • Perform invasive procedures independently upon completion of documented competency.
  • Participate in patient care rounds and conferences, communicating management strategies to the care team.
  • Recognize situations requiring physician intervention and initiate life‑saving procedures when necessary.
  • Use advanced communication skills to problem‑solve complex situations and improve patient services.
  • Collaborate with multidisciplinary team members to analyze and improve health‑care delivery systems.
  • Participate in outside activities that enhance personal and professional growth.
  • Arrange and write orders for SNF discharges, complete paperwork, and coordinate transitions to lower levels of care.
  • Introduce NP role to patients/families, facilitate family conferences, and support shared decision‑making regarding goals of care, palliative care, and hospice.
  • Facilitate discharge to appropriate care levels and use preferred providers for additional services.
  • Prescribe medication to patients based on state practice authority.
  • Perform other duties as assigned and modified by the manager.

Education and Experience
  • ARNP or equivalent advanced nursing degree required.
  • Current state nurse‑practitioner certification required.
  • Board certification by AANP or ANCC preferred, may be required in some states.
  • Current DEA number for schedule II‑V controlled substances may be required.
  • Minimum two years of clinical NP experience in family medicine, internal medicine, or geriatrics with demonstrated autonomy in independent care decisions.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within 90 days of hire.
Pay Range

$111,140 – $158,771. Salary is based on experience, education, and location. The position may also be eligible for bonuses or commissions.
