Home Health Registered Nurse (RN) – Downers Grove, Illinois (Ongoing, 40 hrs/week, 8 hr shift, Staff)

Lead and collaborate to provide management and delivery of patient care plans as a registered nurse case manager in a team-based environment.

• RN license in the state of practice
• Graduate from an approved school of practical nursing
• One year of RN experience with clinical competence to deliver quality patient care
• Current driver's license and ability to spend approximately 20% of your day traveling to patient locations
• Commitment to consistently meet critical deadlines for charting
• Self-management skills to plan and schedule your time effectively
• Demonstrated experience with tablets, mobile phones, and EMR software
• Successful completion of a drug screen (includes medical and recreational marijuana) prior to employment
