Job Description
Be a part of a world‑class academic healthcare system at UChicago Medicine as a Nurse Navigator with our Cardiac Surgery Team in Hyde Park
. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. Under the direction of the Interventional procedure providers and in collaboration with the program support team, you will coordinate the care of patients through their interventional procedure. You will act as a clinical liaison for patients in the clinic and in the in‑basket messages, assess clinical, emotional, spiritual, psychosocial, financial and other patient needs, and evaluate and manage patient pre‑ and post‑operative care.
Job Functions
- Provides care coordination & patient education to patients in the heart and vascular service line.
- Facilitates resolution of patient care gaps and monitors follow‑up/completion by proactively reaching out and addressing direct phone calls and messages from patients.
- Acts as liaison for patients, families and the multidisciplinary clinical team.
- Educates and reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan.
- Develops and/or coordinates patient education materials.
- Serves as primary point of contact for patients discharged from the hospital to address questions and patient care needs to prevent readmission or an ER visit.
- Attends clinic with providers to schedule procedures, testing, and educational information for a successful procedure.
- Attends all program meetings.
- Works in collaboration with other nurses in the Interventional procedure program.
- Performs other duties assigned.
- Registered Nurse with a current Illinois license.
- Minimum of two years of clinical nursing experience with demonstrated competence in patient care, teaching and management of patients in cardiology.
- Must have general computer skills and be adaptable to changing work assignments.
- Excellent interpersonal skills with specific ability to communicate effectively with patients, family members, and physicians.
- Experience in pre‑operative and post‑operative teaching for cardiac procedures.
- Cardiac Surgery experience (inpatient or outpatient).

The range is based on a full‑time equivalent (1.0 FTE) and is reflective of current market data, reviewed annually. Compensation offered at the time of hire will vary based on candidate qualifications, experience and organizational considerations, including internal equity.
