Nurse Practitioner, Home-Based Care – New Jersey

At Clover Health, our Nurse Practitioners bring proactive, compassionate care directly into the homes of Medicare Advantage members who need it most. In this field-based role, you will help members recover safely after hospitalization, manage chronic conditions, close care gaps, and stay connected to the right care at the right time.

We are hiring across North, Central, and South Jersey, including:

• North Jersey:
  Bergen, Essex, Hudson, Passaic, Union, Morris
• Central Jersey:
  Middlesex, Monmouth, Mercer, Somerset, Ocean, Hunterdon
• South Jersey:
  Camden, Gloucester, Burlington, Cumberland, Atlantic, Cape May, Salem

This role is ideal for a Nurse Practitioner who values autonomy, meaningful patient relationships, and the opportunity to make a real impact outside the traditional clinic setting.

You will practice at the top of your license, work independently in the field, and be supported by an interdisciplinary care team. You will also use Clover Assistant, our data‑driven clinical platform, to support decision-making and deliver more proactive, personalized care.

• Conduct in‑home health assessments for Medicare Advantage members
• Help manage chronic conditions and identify clinical changes
• Close care gaps and connect members to follow‑up care and resources
• Collaborate with physicians, care managers, pharmacists, and other partners
• Deliver compassionate, relationship‑centered care in the home

You’ll thrive here if you’re clinically confident, self‑directed, tech‑comfortable, and passionate about caring for older adults and medically complex patients.

• Active and unencumbered New Jersey RN license
• Current New Jersey APN certification as a Nurse Practitioner
• Master’s degree or post-master’s certificate in advanced clinical practice
• At least 1 year of Nurse Practitioner experience
• Active DEA and New Jersey CDS registration, or ability to obtain before start date
• Current BLS certification
• Ability to travel locally for in‑home visits
