We are seeking a compassionate and skilled Home Health Registered Nurse RN to join our team of dedicated healthcare professionals.
Responsibilities- Provide patient‑centered care to patients in their homes, focusing on their unique needs and goals
- Conduct initial assessments, develop and implement individualized care plans, and evaluate patient progress
- Collaborate with interdisciplinary teams, including physicians, therapists, and caregivers, to ensure comprehensive care
- Administer medications, treatments, and therapies as prescribed by physicians and in accordance with agency policies
- Document patient information accurately and in a timely manner, ensuring compliance with regulatory requirements
- Participate in quality improvement initiatives to enhance patient outcomes and agency performance
- Current RN licensure in the state of California
- Minimum 1 year of experience in home health or a related field
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and interdisciplinary teams
- Ability to lift, move, and transfer patients as needed
- Reliable transportation and the ability to travel to patients' homes
