Position Highlights

  • Position:
    Nurse Practitioner (NP) – Post Acute Care
  • Location:

    Skilled Nursing Facilities in Endeavors north region (Evanston, Swedish Hospital)
  • Full Time/Part Time:
    Full‑Time, 40 hours per week
  • Hours:

    Monday – Friday between 8:00 a.m. and 5:00 p.m.
  • Potential for weekend hours in the future
  • No evenings or holidays
What you will need
  • License:
    Nurse Practitioner (APN) Licensure in the State of IL required;
    Federal DEA and Illinois Controlled Substance License required
  • Education:

    Completion of Master’s level program in specialty area
  • Certification: BLS/CPR certification required
  • Experience:

    Minimum of three years Nurse Practitioner experience. Experience with advanced directive and goals of care.

  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
  • Promote wellness and prevent illness and injury in various settings for individuals, families, and communities
  • Work autonomously and in interdisciplinary teams as resources and consultants
  • Positively contribute to safety outcomes and promote high quality patient experience
