Registered Nurse – Home Health
Illinois Home Health, part of LHC Group, provides post‑acute care in patients’ homes. As a Registered Nurse, you will provide and direct nursing care, coordinate care plans, and ensure compliance with laws and agency policies.
Primary Responsibilities- Clinical Competence
- Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.
- Supervise care provided by home health aides and licensed practical/vocational nurses, provide instruction, and assign tasks according to State and federal regulations.
- Provide required supervisory visits.
- Documentation and Care Delivery
- Provide high‑quality clinical services within scope of practice and infection control standards, coordinating with other team members.
- Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
- Document patient visits per policy and payer requirements, and sync timely per LHC policy.
- Quality & Teamwork
- Make initial and comprehensive nursing evaluation visits, ensuring patients meet eligibility and medical necessity.
- Communicate timely and effectively with agency staff, physicians, patients, and family members.
- Take direction from Clinical Director and Executive Director, completing assigned tasks timely.
- Assist in orientation of new personnel, serve as preceptor, and participate in survey readiness and performance improvement activities.
- Participate in on‑call and weekend rotations as needed.
- Adhere to the agency's utilization management model.
- Function in any home situation regardless of client background.
- Current and unrestricted RN license in the state of practice.
- Current valid Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification or ability to complete within 90 days of hire.
- Ability to work independently.
- Solid communication, writing, and organizational skills.
Target Pay: $79,222 – $118,833
Per Visit Point Pay Range: $38.09 – $57.13
Hourly Pay Range: $45.71 – $68.56
Annual total cash compensation assumes full‑time employment (40 hours weekly). Compensation includes per visit point and hourly rates; benefits and bonuses may vary by location and performance.
