Foundation Health Partners invites a Nurse Practitioner to join our Outpatient Internal Medicine & Senior Care Clinic. In this role you will provide services to patients with chronic and multisystem disorders, collaborate with a dedicated care team, and contribute to our mission of patient‑centric care.

• Compensation: Starting at a base salary of $147,112, with possible adjustments based on experience and productivity; $40k sign‑on bonus.
• Schedule: Monday‑Friday, 40 hours per week; 7 weeks of annual time off, 2 weeks of CME.
• Patient panel: Diverse acuities and populations.
• Team: We operate under a Care Team model, where APPs handle routine check‑ins, follow‑ups, and acute care, while physicians provide consultative services and manage complex cases.

Minimum Qualifications

• Master’s degree in nursing with knowledge of clinical care and patient treatment.
• Approved to practice as a Nurse Practitioner by the State Board of Nursing and current RN license in the state of practice.
• Certification to practice by the State Board of Nursing in a designated population as a nurse practitioner, and personal DEA license or state‑agreed prescribing authority.
• Exemplary professional demeanor and interpersonal skills.
• Proficiency in computer applications and database management.

Preferred Qualifications

• DNP or PhD preferred.
• Current BLS certification preferred.
• At least 3 years of previous relevant experience preferred.
