Job Title: Nurse Practitioner

Location: Fairfield, CA

Remote Status: Hybrid

Schedule: Monday-Friday with flexibility based on patient and partner needs

The Role
  • Support highly complex patients, assisting them in navigating a challenging healthcare system and providing one‑on‑one care they need and deserve.
  • Provide patient education, create dedicated care plans, and give clinical guidance to an integrated care team with the patient’s best interest as the guiding principle.
  • Work closely with the Integrated Care team to provide superior care for our dialysis patient population.
  • Function autonomously and collaboratively with all team members to coordinate and facilitate high‑quality, cost‑effective care.
  • Provide coordinated care services between the patient, family, physicians, additional providers, and other team members.
  • Administer various tests and screenings.
  • Perform comprehensive health assessments and health risk assessments with a focus on thoughtful, accurate, and specific clinical coding and documentation.
  • Manage longitudinal care of an assigned patient panel of CKD and ESKD patients, including timely post‑discharge follow‑up, utilization management, readmission prevention, and medication management.
  • Maintain strict confidentiality in line with HIPAA regulations.
What You Can Expect

This full‑time Nurse Practitioner opportunity includes a mix of in‑clinic (dialysis centers and nephrology practices), in‑home visits primarily within your designated service area, occasional telehealth appointments, and flexible coverage across regions as needed.

Experience and Qualifications
  • Nurse Practitioner certification from either the ANCC or AANP.
  • 2+ years of clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Nephrology, Gerontology, or Palliative Care; home care experience is a plus.
  • Working knowledge of chronic care management and various medication options.
  • Current DEA licensure / Prescriptive Authority post‑hire.
  • Active CPR/BLS Certification.
  • Knowledge or experience working with HCCs.
