Schedule:

Must be available full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full time:
Saturday and Sunday plus 3 weekdays.

Anticipated territory:
Solano County area.

• Conduct Start of Care (SOC) admissions – OASIS.
• Conduct follow‑ups and evaluation visits as needed within the designated territory.
• Make home visits to patients in the territory.
• Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures.
• Monitor clients’ conditions, reporting changes to the clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively per protocol.
• Accurately document observations, interventions, and evaluations related to patient care management.

• Concierge support for Case Managers with administrative tasks.

• Current, unrestricted California RN license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• Experience conducting assessments and SOCs (OASIS) is highly desired.
• High level of clinical knowledge in home health care preferred.
• Experience with high acuity patients is a plus.
• Experience with interdisciplinary teams is highly desired.
• Fundamentals of case management desired.
• Previous experience with IV medication infusions preferred.
• Proficiencies in initiating IV therapy a plus.
