We are looking for a experience nurse to work very closely with both the clinic and hospital teams in ensuring that patients surgical care journey is seamless. This includes; teaching a total joint class to patients, preparing for total joint surgeries, reaching out to these patients by phone after they go home, as well as rounding on them if they stay in the hospital to ensure clear understanding of steps in the process and peri operative directions and follow up.
Provides input with patient education materials/booklets, coordinates communication across teams and departments with focus on ensuring positive and excellent patient experience and clinical outcomes.
Delivers professional nursing care in the ambulatory setting to patients as identified for the assigned clinical program. Performs the primary functions of a Registered Nurse within a department, inclusive of assessing, diagnosing, planning, implementing, and evaluating the care of all assigned patients. Works with professionals who provide services with the intention of building capacity and maintaining quality of services.
Job Responsibilities and Requirements Primary Responsibilities- Serves as a liaison for management staff, administration, providers, managed care companies, community organizations, and other customers.
- Monitors and maintains regular contact with patients in the caseload as identified by physicians. Performs pre-visit chart reviews on the caseload patients. Identifies labs to be done, prescriptions to be refilled, consultations that may be due, etc. and ascertains that all appropriate preventive care is being performed.
- Coordinates outreach processes for the patients with care opportunities identified by a provider, including visits, consults, lab, x-rays, ancillary services, and appropriate community resources.
- Serves as point of contact for all patients within the service to support discharge plan; medication review and schedule follow up appointments with the provider.
- Provides referral/support for follow-up care for patients who are uninsured or underinsured.
- Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.
- Educates patients about self-management tasks they can undertake to gain greater control of their health status and coordinates with physician and care team workflows to involve the patients in activities to improve their health. Establishes working relationships teaching protocols for patient education, family counseling, and community information.
- Works with the practice leadership to implement evidence-based practice guidelines as outlined by clinic policies and providers.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
- Graduate of accredited school of nursing or education equivalency for licensing
- Two years' registered nurse experience
- Constant use of speech to share information through oral communication.
- Constant standing and walking.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, reaching and keyboard use/data entry.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of smell to detect/recognize odors.
- Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Occasional lifting/moving of patients.
- Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
- Occasional driving.
- Rare crawling and running.
