Provides diabetes education to patients, caregivers, staff, and the community.

Responsibilities
  • Provides diabetes self-management education and training to patients with diabetes and their family members utilizing curriculum approved by the American Diabetes Association.
  • Works collaboratively with multi-disciplinary diabetes care team to implement plan of care.
  • Reinforces diabetes education and instruction to staff nurses and support staff.
  • Develops and distributes educational materials to community.
  • Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
Requirements
  • Bachelor's degree in Nursing.
  • State licensure as a Registered Nurse (RN). Preferred: CDCES (Certified Diabetes Care and Education Specialist) as defined by the Certification Board for Diabetes Care and Education (CBDCE) or board certified in Advanced Diabetes Management (BC-ADM) by the Association of Diabetes Care and Education Specialists (ADCES).
  • 2 years nursing experience.
