Responsibilities include all aspects of coordinating care for individuals receiving Targeted Case Management Services to address Behavioral Health (BHCM).
This position will ensure that linkages are made between, health care, behavioral health provider agencies, and other state managing entities. The Behavioral Health Case Manager supports individuals served and their family members through the collaboration, linkage and monitoring of services in the community as deemed appropriate based on the clients initial and on-going assessments.
The BHCM primary focus is mental illness and/or substance abuse diagnosis.
Specific Duties and Responsibilities
1. Develops partnerships with the array of system representatives, to facilitate appropriate and necessary services that are person-centered for clients.
2. Participates in relevant community meetings to obtain and gather relevant information and resources.
3. Coordinates and recommends and facilitates linkages to appropriate services.
4. Makes recommendations in terms of services to meet system needs.
5. Monitors and reports on progress for clients as it relates to progress toward agreed upon service plan goals and outcomes.
6. Conducts case reviews with providers and supervisor as needed.
7. Develop comprehensive assessment and service plans on individuals served; maintain files on individual cases, meetings, and other activities.
8. Collaborates with other care coordinators and support team.
9. Meets regularly with client and other care coordinator and supervisor to stay abreast of issues.
10. Other duties, as requested.
Experience and Education
v Minimum of a Bachelor’s Degree in Psychology or a related field.
v Behavioral Health Case Manager Certification (BHCM) or equivalent preferred.
v Minimum of 1 years’ experience in the mental health or case management with the target population.
v Will be required to complete the Florida Certification Board Behavioral Health Case Management Certification Training within 6-months of hire date.
To succeed as a Behavioral Health Case Manager the individual requires the following:
1. Understanding of the behavioral health system and court system in Broward County.
2. Understanding of the overall system of care.
3. Strong communication skills, both verbal and written.
4. Proficient with computer programs such as Microsoft Office.
5. Documentation – Skilled in writing notes and other reports.
6. Time Management- The ability to plan accordingly and submit progress notes and other required documents timely
7. Judgment – Displays willingness to make decisions, exhibits sound and accurate judgment and makes timely decisions based on sound ethical practices.
8. Empathy – Displays appropriate empathy for persons with problems and problem-solving skills to assist in the development of necessary and appropriate services.
9. Diplomacy – Able to deal effectively and positively with staff and external parties including providers and community stakeholders.
10. Follow Through – Demonstrates the ability to assume responsibility for tasks and insure they are completed thoroughly and timely.
11. Understanding of Data/Outcomes as they relate to the target population to be served.
Must have reliable transportation
Has a valid driver’s license and acceptable driving record. Will consent to background screening processes as required by the State, based on holding this position of special trust to include fingerprint-based search of criminal records in Florida and nationally.
