Overview
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our ultimate mission is to dramatically improve patient access to home health care.
The Field Supervisor (RN) role is how we scale that mission: you guide newly hired and transitioning clinicians through real-world home health practice, validate their clinical competency, and model the standard of care every patient deserves.
What You Will Do- Time allocation: 75% field work (independently or with new joiners), 15% dedicated education time at your local branch, 10% special projects (e.g., Fira focus groups, improving education resources)
- Precept and coach newly hired clinicians during patient visits, providing real-time clinical guidance and feedback
- Provide hands-on mentorship and support to new joiners — with particular focus on clinicians with no prior home health experience
- Evaluate and sign off on new joiners clinical competency through structured observation during patient visits
- Assist with skills lab training sessions and other structured onboarding activities
- Administer and support end-of-orientation assessments for new hires
- Serve as a resource and primary point of contact for new joiners throughout their orientation period
- Maintain a reduced but active patient caseload to stay current with clinical practice while dedicating meaningful time to precepting and teaching
- Model quality standards in OASIS documentation, clinical decision-making, and adherence to Conditions of Participation (CoP)
- Identify and escalate concerns related to new joiner readiness or patient safety to the Clinical Manager
- Participate in Fira improvement focus groups, contributing frontline clinical insight to refine and enhance Adaptive s tools and workflows

Salaried, exempt - RN: $99,840 annualized at 27 points/week expectation
- Mileage and travel reimbursement in addition to base

- Active, unrestricted Texas RN license
- Minimum 2 years of home health experience (current or prior employer)
- Clean disciplinary record — no adverse action in the last 12 months
- No licensure encumbrances within the last 5 years
- Current CPR/BLS certification
- Current Texas driver s license and comfort with field-based work
- Strong clinical assessment skills and OASIS knowledge
- Patient, encouraging communication style with the ability to give constructive feedback
- Paraprofessionals (LVN/LPN) are not eligible
- Prior experience as a preceptor, clinical mentor, or educator in any setting
- COS-C certification (or willingness to obtain — we cover the cost)
- Familiarity with Homecare Homebase (HCHB) and Point Care
- OASIS-E2 trained, HCS-D, or wound care certification
- Experience training clinicians who are new to home health
