Nurse Practitioner – Wound Care Job Summary – Nurse Practitioner (Wound Care)

Dr. Mobi Care is seeking a licensed Nurse Practitioner with experience or interest in wound care to join our clinical team. This role focuses on delivering high-quality, patient-centered care while collaborating with a multidisciplinary team to support healing and overall wellness.
Key Responsibilities- Perform comprehensive physical exams and wound assessments for patients with acute and chronic conditions
- Evaluate wound-related conditions and comorbidities to support optimal healing outcomes
- Perform wound debridement and dressing changes in accordance with clinical best practices
- Develop and implement individualized care plans in collaboration with the Medical Director and care team
- Accurately document patient encounters, procedures, and communications in the EMR within 24 hours
- Educate patients and caregivers on wound care, prevention strategies, and chronic condition management
- Communicate effectively with primary care providers, specialists, and interdisciplinary teams
- Collaborate with clinical staff and leadership to maintain efficient workflows
- Participate in clinical meetings, training, and quality improvement initiatives
- Advocate for patients by ensuring access to appropriate care and follow-up services
- Maintain compliance with clinical, regulatory, and safety standards
- Stay current with CMS guidelines, documentation requirements, and best practices
- Maintain licensure and pursue ongoing education in wound care and related specialties
- Perform additional duties as assigned
- Active, unrestricted Nurse Practitioner license in applicable state(s)
- Clinical experience in wound care or a related specialty preferred but will train
- Strong skills in debridement, and care planning
- Excellent communication and documentation skills
- Ability to work independently and collaboratively within a clinical team
- Monday–Friday schedule (no weekends)
