Job Description

Schedule: Full‑time;
Flexible shifts ranging from 8‑12 hours per day depending on facility needs

Provide compassionate resident care and daily support within a collaborative long‑term care environment serving residents with skilled nursing, rehabilitation, mental health, and memory support needs. Working alongside nurses, rehabilitation professionals, and interdisciplinary care teams, this Certified Nursing Assistant (CNA) role supports resident comfort, safety, mobility, and dignity within a professionally managed, stable long‑term care environment.

This role offers the opportunity to build meaningful resident relationships while contributing to a supportive care team focused on consistency, compassion, and quality of life.

Key Responsibilities

  • Support residents with daily living activities that promote comfort, dignity, mobility, and independence.
  • Observe resident conditions and communicate changes or concerns to licensed nursing staff.
  • Assist with specimen collection, resident preparation for activities, and routine clinical support tasks.
  • Help maintain a safe, comfortable, and supportive care environment focused on dignity and consistency.
  • Collaborate with interdisciplinary healthcare teams to support individualized resident care plans.
  • Accurately document care activities and assigned resident information as directed.
Qualifications

Requirements

  • Graduate of a Medicaid‑approved Nursing Assistant program with certificate of completion.
  • Active Kentucky State Registered Nurse Aide certification required.
  • Basic Life Support (BLS).
  • High School Diploma or GED required.
  • Ability to meet facility onboarding and screening requirements.
  • Ability to support resident mobility, positioning, and daily care activities in a long‑term care environment.

Preferred

  • Prior experience in long‑term care, rehabilitation, behavioral health, or skilled nursing settings.
  • Experience working within collaborative interdisciplinary healthcare teams.
  • Previous experience supporting residents with memory care or mental health needs.

Compensation Disclaimer: Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant’s background, experience, skills, internal equity, and alignment with geographical market data.
