The Care Transition Nurse Manager (RN) is responsible for coordinating and managing all admissions for the agency.

As a member of the Care Transition Nurse Team, you will serve as a critical liaison between the benefit of hospice services and potential patients, their loved ones and caregivers, and our healthcare partners. The Position:

The Care Transition Nurse Manger is responsible for coordinating and managing all admissions for the agency. The Care Transition Nurse Manger plans, organizes and directs all needs assessments and admissions related task to ensure that the agency remains compliant and provides quality care. This person will assist admissions staff on overcoming objections, attends and leads morning huddle as well as track and trend conversion rates. He/she will assist in tracking non-admitted patients who are pending so patients may be admitted as soon as appropriate to achieve targeted growth and oversee the orientation and continued education of all admissions staff along with RN Case Managers

The Requirements:

  • Graduate of an accredited school of nursing with an associate degree required, BSN degree is preferred Must be a Registered nurse.
  • RN with current licensure with no restrictions to practice professional nursing in the required state(s).
  • Must have Two (2) years’ as an RN in healthcare
  • 2 years’ experience in both the hospice and hospital setting preferred.
  • Proven ability to work within an interdisciplinary setting.
