Shift: 16 hours Sunday – Monday, 8 AM – 4 PM. Salary: $19.39 hourly; weekend shift differential of $1 per hour ($20.39). Position:

Scope of Service

Assist individuals with daily personal routines, providing prompts for daily living skills, skill‑building activities, and group participation. Provide companionship, establish a trusting professional relationship, and work with nursing personnel to monitor medical concerns and medication usage. Report any concerns or medical issues; ensure living spaces remain safe, sanitary, and well‑organized. Lead group activities and outings, and collaborate with nursing, management, and coworkers to ensure residents receive the highest quality services.

ESSENTIAL

JOB FUNCTIONS
  • Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Enhance clients’ independent living skills through engaging, assessing, and applying various teaching modalities (modeling, prompting, hands‑on assistance).
  • Provide hands‑on support for activities of daily living such as personal care/hygiene, housekeeping, laundry, food management/nutrition, budgeting, health and safety, leisure, and other activities promoting healthy living.
  • Facilitate/co‑facilitate psycho educational groups as indicated.
  • Engage clients in community integration and social recreation activities.
  • Provide transportation to and from appointments; educate clients on travel planning and using public and private transportation.
  • Inspect apartments/rooms regularly and assist clients in maintaining the cleanliness of their living spaces.
  • Conduct ongoing rounds to ensure safety of each individual and the program.
  • Administer breathalyzer tests and urinalysis samples, as applicable.
  • Document shift logs in electronic record according to contract and agency policy.
  • Develop and maintain working relationships with providers, families, and natural supports.
  • Respond to crisis situations and provide interventions as outlined in the agency policy for Nonviolent Practices.
  • Supervise, monitor, and document self‑administration of client medications.
  • Attend and participate in monthly staff meetings, clinical rounds, All Treater meetings, and supervision.
Environmental Responsibilities
  • Maintain a clean, well‑organized work environment.
  • Ensure all program egresses and walkways are clear to support workplace safety.
  • Notify supervisor of maintenance and/or safety issues and generate work orders as needed.
  • Conduct and participate in fire and emergency evacuation drills, and review the emergency preparedness plan as outlined in agency policy.
  • Participate in Food Bank activities as assigned.
  • Remain awake during all shifts to maintain the security and safety of clients and the facility.
Quality Assurance
  • Adhere to policies and practices outlined in the staff handbook and program operations manual.
  • Consistently work in a respectful, constructive, and cooperative manner.
  • Uphold HIPAA regulations and secure all necessary releases of information for outside communication.
  • Model culturally competent behavior when interacting with clients, providers, family, and fellow staff.
  • Maintain accurate electronic records and case files in accordance with programming, contractual, licensing, and accreditation requirements.
  • Adhere to CARF standards.
  • Participate in agency committee work when available or as permitted.
  • Complete all trainings as required per program contract and agency policy.
  • Attend agency‑sponsored in‑services, outside trainings, conferences, and meetings to facilitate professional growth and ensure quality case‑management practices.
QUALIFICATIONS
  • High School Diploma or equivalent considered with a minimum of one year experience in a related healthcare field. Licensed CNA or other healthcare certification accepted in lieu of experience.
  • Prior experience working with people with severe and persistent mental illness, intellectual disabilities, and/or addiction.
  • Knowledge of the community support system and resources preferred.
  • Valid Connecticut Driver’s license with a clean driving record (required).
