Job DescriptionEnnoble Care is seeking a full‑time experienced Care Coordinator, Embedded to work at our client’s senior living facility in Hamilton / Florence / Echelon, NJ. The role ensures high quality care services, coordinating with clinicians, staff, and patients to achieve healthcare goals and maintaining open communication.
Responsibilities- Complete individualized patient care plans and provide care management and coordination services using Ennoble Care’s electronic medical record system
- Frequent contact with patients to provide care coordination, support, and manage compliance with care management programs to improve outcomes
- Document all client communications accurately
- Communicate with Primary Care Clinician or designee regarding patient emergent needs, ensuring comprehensive care plans are complete and accurate
- Keep Team Supervisor informed of all issues pertinent to the care plan process
- Work with cross‑organizational areas to meet the needs of patients, families, and partner facilities
- Use technology such as secure email, telephone system, e‑records
- Adhere to documentation protocols and best practices for daily logs, escalation of client issues, and internal communications
- Provide excellent customer service, demonstrated by positive feedback from patients and customers
- Contribute as a positive team member, supporting all members constructively
- Utilize computer, telephone, copy machine, and other office equipment as necessary
- Comfortable speaking on the phone for large amounts of the day
- Compassionate and empathetic toward patients, demonstrating exceptional customer service
- Accuracy in note‑taking and documentation
- Multi‑tasking between patients and work streams while remaining organized and efficient
- Ability to thrive in a fast‑paced environment
- Full‑time availability Monday through Friday during daytime hours at the facility
- Proficiency with computer and Microsoft Office (Outlook, etc.)
- Knowledge of basic healthcare terms, conditions, roles, and care principles
- Candidate must pass drug screen, background check, display a positive attitude, adapt to change, and be a team player willing to learn continuously
