In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Grand Junction

Overview

Signify Health, part of CVS Health®, is hiring a part‑time Clinician—Nurse Practitioner or Physician Assistant—to provide In‑Home Health Evaluations. The role involves visiting patients in the comfort of their own homes, building trusted relationships and delivering personalized health assessments.

• Conduct approximately 45‑minute in‑home evaluations, including medication and medical history review, physical assessment, and additional diagnostic tests such as diabetic eye exams or spirometry when ordered by the health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation, without prescribing medication, ordering lab tests, or altering existing treatment regimens.
• Travel locally and within licensed states as required; conduct virtual visits when necessary.
• Provide all Signify Health Diagnostic & Preventive Services products (e.g., spirometry, DEE) and other services indicated by the health plan.
• Maintain safety and quality standards while working in diverse home environments that may include pets, secondhand smoke, or other allergens.

Primary workplace is the private residence of health‑plan members. Clinicians must be comfortable and safe performing duties in varied home settings.

• Active, unrestricted NP or PA license(s) in the coverage area(s).
• Board certification required; multi‑state license preferred.
• Ability to cover Grand Junction and surrounding areas.

Part‑time: 29.5 hours per week. Pay range $41.85 – $90.13 per hour, depending on experience, education, geography, and other factors.
