As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary.
Their main focus is advocating for patients, families, and caregivers while embodying the organization's vision, mission, and values. The goal is to give an unmatched patient experience and attain optimal clinical outcomes.
- Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
- Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
- Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.
- Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
- Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
- Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
- Demonstrates a time-conscious awareness and ability to prioritize. Distinguishes between low and high-priority activities.
- Graduated from an accredited school of nursing with an Associates or Bachelor of Science in Nursing (BSN) is preferred.
- Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 60 days of hire date).
- American Heart Association BLS Certification is required.
- 1-year of clinical Med Surg experience as a licensed RN in an acute care setting required - [Observation, OBs, Chest Pain, Short Stay, EDOU, ED Observation, CDU, Clinical Decision Unit, Admission, Discharge]
