Position Summary and Job Duties
This position involves the implementation of activity programming for Rock Haven residents. The activities are conducted within the recreational therapist's organized plan, requiring initiative, creativity, and considerable judgment.
Schedule:
7:00 a.m. – 3:30 p.m. (every other weekend and every other Wednesday, Thursday). Shift . Full time, including each other weekend shift rotation.
- Review "Assessment & Care Planning Meeting Schedules".
- Review resident's POC.
- Possess knowledge and understanding of individual resident's goals and approaches.
- Assist with modifications to the POC.
- Interview residents at least quarterly for leisure satisfaction.
- Write quarterly progress notes.
- Collect and record documentation as requested (participant attendance, observation flows, behavior flows, injury/incident reports, etc.).
- 25% – Prepare documentation.
- 15% – Prepare group programs.
- 15% – Deliver group programs.
- 10% – Housekeeping tasks associated with activities.
- 15% – Deliver individualized programs.
- 15% – Department operations tasks.
- 5% – Other duties as assigned.
- Ability to reach out to residents with warmth, compassion, and patience, to put residents at ease, and to engage them quickly.
- Ability to communicate clearly, verbally and in writing, with co‑workers, residents, and supervisors.
- Planning and organizing activities, assembling supplies, and managing event details.
- Ability to provide a multifaceted activity program that includes recreational and leisure activities.
- Adaptation of activities to the resident’s functional level and needs.
- Knowledge of community resources relevant to leisure programs.
- Provide ADL and mobility assistance to residents (feeding, transfers, toileting, ambulation).
- Problem‑solving and common‑sense risk recognition.
- Document participation; participate in activity assessments.
- Guide and support volunteers and delegate appropriate tasks.
- Flexibility and independence in work.
- Perform other reasonably related duties as assigned.
- Note:
The entire Rock Haven Campus is smoke‑free.
- Graduation from a standard high school or equivalent.
- Prior experience with elderly clients, clients with mental illness, or clients with developmental disabilities as an activity assistant or nursing assistant.
- Certified Nursing Assistant certification is required. Activity Assistants will be assisted to maintain that certification.
- Attend mandatory facility in‑services.
- Valid Wisconsin driver’s license.
- May need to stay past the normal shift until relieved in order to meet minimal staffing requirements.
The ATA must be able to lift 15 pounds regularly throughout a shift.
Must be able to push a resident who may weigh up to 300 pounds 50–300 feet in a wheelchair.
Meet other CNA requirements.
Work indoors in a county nursing home.
