Overview
The salary range for this position is $37.00 - $59.20 (Hourly Rate). Placement within the salary range is dependent on relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

The Registered Nurse embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under the direction of the department leadership, consistently reflects the mission, vision, and values of NM, adheres to the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Home Hospital Nurse treats acutely ill patients in the home as a substitute for in-hospital care. The Home Hospital Nurse is a forward-thinking, creative individual who wants to practice in an innovative clinical setting and values interdisciplinary teamwork and patient-centered care. The Nurse will provide hospital-level acute care in a patient’s home and will be part of the NM Hospital at Home team including Physicians, Paramedics, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Home Health Aides, Physical Therapists, and other operational staff.
Provides home visits within defined radius around site hospitals according to Hospital at Home care Model.
RESPONSIBILITIES:
- Performs a comprehensive assessment on all assigned patients as evidenced by completed documentation.
- Communicates changes in patient’s condition promptly.
- Utilizes appropriate nursing diagnosis and data from the assessment to derive realistic outcomes and documents them as measurable goals on the plan of care; modifies care plan, if indicated, in order to identify realistic patient goals.
- Identifies and prioritizes patient needs and/or nursing diagnosis for patients and documents.
- Collaborates with patient, family, and other members of the health care team when planning patient care and documents accordingly. Assesses patients with regard to discharge planning and makes the appropriate referrals as necessary.
